Replace the belt tensioner, not just the pully. Maybe the waterpump, it might be siezed intermittenly. Really any accessory running on the belt can cause it. you could just run it with the hood up and listen for broken bearings. Its most likely the tensioner.

Check the tension, if it's too tight it can break.
Also check the pulley alignment, if it's off a little the belt can run onto the edge and get cut .
LOOK for a bad pulley
take off the belt then feel all of the pulley wheels for sharp edges
next make sure that the belt is not binding on any thing.
